The completed AD-3027 form or letter must be submitted to USDA by:

INFORMATION

A new free and reduced lunch application needs to be filled out by ALL students who qualified last year in order to receive those benefits this year.  These DO NOT transfer from your home school.  Please send money with your student on the first day of school if you did not fill out a new application at Vantage or have not yet received verification of qualification.

If you would like to print off some of these deposit slips, your student may bring them to school filled out and ready to turn in with payment during breakfast hours.
